    Registration procedures

    Kindergarten

    Kindergarten  Eligibility Date 
    Since the 2009-2010 school year, students attending kindergarten must be 5 years of age on or before August 31. 

    EARLY ADMISSION: If you are interested in pursuing early admission to Kindergarten, please read the State policy.

    Most parents choose to enroll their 5-year-olds in kindergarten even though the state does not require children to be enrolled in school until the age of 7.

    To register your child, please go to your child’s school. If you do not know which school your child will be attending, go to the School Assignment Locator page. Your child does not have to be present to be registered, but you will need to take along the following:

    • A certified copy of the child’s birth certificate, which can be obtained from the county or state health department where the child was born, or other satisfactory evidence of date of birth.
    • (A) One (1) such proof must be a signed lease agreementaffidavit of an oral lease agreement, deed or closing statement (with a closing date within 45 days) in the name of the parent/legal guardian(s).
      Other proofs of residency to meet this requirement may include:
      • Property tax bill showing both the owners mailing address and the property address;
      • Home visit by a Lee County Schools social worker. Two proofs of residence one such proof from category (A) and one such proof from category (B).

       

    • (B) The second proof of residence may include one of the following:

      • Current utility bill for the residence (electric, water, gas) in the name of the parent/legal guardian(s). Phone and cable bills are not accepted;
      • Property tax bill showing both the owners mailing address and the property address;
      • Home visit by a Lee County Schools social worker (can be used for A or B).

       

    • (C) If an affidavit* is used as a rental agreement it does not need to be submitted each year. A new affidavit used as a rental agreement must be submitted when the student moves from one school configuration to the next.

    • (D) If there are multiple children residing in the home with the same residency information they may all be listed on one affidavit. All required information must be presented to the principal of the school at the time of enrollment. If documents are not available parents/legal guardian(s) will be given 15 days to submit all required paperwork.

    • An updated immunization record. Medical evidence of the following immunizations must be submitted:
      • Five DPT doses (diphtheria, tetanus, and pertussis). If the fourth dose (booster) is given on or after the fourth birthday, the fifth dose is not required.
      • Four polio vaccine doses (IPV). If the third dose is given on or after the fourth birthday, the fourth dose is not required.
      • Two Varicella (chickenpox) vaccine.  (Physician and/or parent may document "Had disease")
      • Two MMR (measles, mumps, rubella) doses with the first dose given on or after the first birthday and a second dose given before entering school.
      • At least one dose of Haemophilus influenza, b (HbOC, or PRP-OMP), given on or after the first birthday and before five years of age.
      • Three Hepatitis B doses. This vaccine is required of all children born on or after July 1, 1994.
    • North Carolina law requires that a kindergarten health assessment be completed on or before the first day of school before a child can enter kindergarten. The health assessment must be completed no more than 12 months prior to the Kindergarten start date.
      • If a completed health assessment form is not presented on or before the first day, the principal shall present a notice of deficiency to the parent, guardian, or responsible person. The parent, guardian, or responsible person shall have 30 calendar days from the first day of attendance to present the required health assessment form for the child. Upon termination of 30 calendar days, the principal shall not permit the child to attend the school until the required health assessment transmittal form has been presented. Health assessment forms are available at Lee County Schools, area doctors’ offices or at the Lee County Health Department. Parents whose children do not have a regular physician can get health assessments and immunizations through the Lee County Health Department.

    Schools are staffed all year and throughout the summer. Please register your child as early as possible so the school will be prepared for your child. A special registration period for entering kindergartners is held in the spring for the following school year, but newcomers arriving after that date can register any time.

    *Use of Affidavits
    Affidavits may be considered as provided under N.C.G.S. 115C-366 as follows: 

    • Educational Residency Affidavit - Parent or Legal Guardian(Blue form) completed and signed by the parent/legal guardian(s); and/or 
    • “Educational Residency Affidavit - Custodial Adult/Landlord(Gray form) completed and signed by the person with whom the family resides and who does have proof of residency in Lee County (two of the items listed above)

    Affidavits must be notarized by a Notary Public whose appointment is current. The Notary Public cannot be a Lee County Schools employee. Upon completion, forms must be submitted to the school's principal for review and consideration. Should the principal have procedural questions, the principal is to consult with the Assistant Superintendent of Auxiliary Services prior to approval or denial. Affidavits are not to be considered final or permanent enrollment tools. New affidavits are required each year unless used as a rental agreement.

    Students Residing with Custodial Parent/Legal Guardian
    Students who wish to transfer within Lee County Schools, from one Lee County school to another Lee County school, must reside with the custodial parent/legal guardian(s). Students and the custodial parent/legal guardian(s) must reside at the same residence in the desired school zone. If legal guardianship/custody has not been obtained, the student must remain at the current school until a custody order has been issued by a court of competent jurisdiction. The permanent criterion is considered to be the custodial order issued by the presiding judge. The above listed proofs of residency are still required.


     

    IMMUNIZATION & HEALTH ASSESSMENT INFORMATION

    2017-2018

    1.  Certificate of Immunization must include:

    • Name and date-of-birth of child.

    • Name of the parent(s) or guardian.

    • Address of the parent(s) or guardian.

    • Sex of the child.

    • Date (month-day-year) of each dose of vaccine administered.

    • Must be signed or stamped by physician or clinic office.

    A printed record from the North Carolina Immunization Registry will meet the requirements for the necessary documentation.


    2.  Immunization Requirements for Kindergarten

    5 DTP (diphtheria, tetanus, pertussis) vaccine with the 4th or last dose after the fourth birthday.


    4 Polio vaccine doses (oral polio vaccine) or IPV (inactivated polio vaccine). If the third dose is given on/after the fourth birthday, a fourth dose is not required. The 4th/last dose of polio must be received be on/after the fourth birthday.


    2 MMR (measles or rubeola, mumps and rubella) vaccines with the first (measles) dose on/after the first birthday.


    1 Hib (Hemophilus Influenza b) after 15 months of age, but not required after age 5.


    2 Varicella vaccine doses, only the physician may document “Had disease.”


    3 Hepatitis B vaccines.


    3.  Additional Requirements


    A Health Assessment is required of all students (k-12) entering North Carolina public schools for the first time. It is due within the first 30 calendar days after they enroll and must have been done within 365 calendar days of their enrollment date. Students will be excluded if they do not have the Transmittal form completed and returned to school within the 30 calendar days.

     
     

    INFORMACIÓN SOBRE LOS REQUISITOS DE VACUNACIÓN Y EXAMEN DE SALUD

    2017-2018

    1.  El Certificado de Vacunación debe incluir:

    • Nombre y fecha de nacimiento del niño.

    • Nombre del(los) padre(s) o tutor.

    • Dirección del(los) padre(s) o tutor.

    • Sexo del niño.

    • Fecha (mes-día-año) de cada dosis de vacuna recibida.

    • Firma o sello del médico o clínica.

    Una copia impresa del Registro de Vacunación de Carolina del Norte (North Carolina Immunization Registry) cumplirá con los requisitos de documentación.


    2.  Requisitos de Vacunación para los estudiantes de Kindergarten/Jardín de Infantes

    5 dosis de DPT (difteria, tos convulsa y tétanos), con la 4º o última dosis dada una vez cumplidos los cuatro años.


    4 dosis de la vacuna OPV (vacuna polio oral) o IPV (anti poliomielítica inactivada). Si la 3º dosis fuera dada al, o luego de, cumplir los cuatro años, no se necesitará una 4º dosis. La 4º o última dosis de polio debe darse al, o luego de, cumplir los 4 años.


    2 dosis contra el Sarampión, las Paperas y la Rubéola (MMR), con la primera dosis contra el sarampión dada al, o luego de, cumplir el primer año de vida.


    1 Hib (Haemophilus Influenzae tipo b) luego de los 15 meses de edad, aunque no es requerida luego de los 5 años de edad.


    2 dosis contra la Varicela, Sólo el médico puede documentar “tuvo la enfermedad”.


    3 vacunas contra la Hepatitis B.


    3.  Requisitos Adicionales


    Todo estudiante que se inscribe por primera vez en las escuelas públicas de Carolina del Norte entre kindergarten y el 12º grado, debe presentar un Examen de Salud. El mismo debe haber sido hecho dentro de los 365 días calendario de la fecha de inscripción, y se tendrán 30 días calendario para presentarlo. De no cumplir con este requisito dentro de los 30 días permitidos, el estudiante será separado de la escuela.



    First-Grade Through Twelfth

    Newcomers and those who have moved to a new school attendance area in Lee County should bring the following items when registering a child:

    • A certified copy of the child’s birth certificate, which can be obtained from the county or state health department where the child was born, or other satisfactory evidence of date of birth.
    • (A) One (1) such proof must be a signed lease agreement, affidavit of an oral lease agreement, deed or closing statement, (with a closing date within 45 days) in the name of the parent/legal guardian(s).

      (B) The other proof of residence may include one of the following in the name of the parent/legal guardian(s) with address and a current date:

      • Utility bill for the residence (electric, water, gas)
      • Property tax bill
      • Home visit by Lee County Schools' social workerTwo proofs of residence one such proof from category (A) and one such proof from category (B).
    • Immunization records.
    • If coming from another school system, a copy of the child's latest report card and the previous year's achievement test results are helpful.
    • Please call the school to make an appointment to register your child. While not required, this can save you a lot of time.
